Swanton Care & Community is looking for an enthusiastic and motivated Scheme Manager to lead our new supported living service in Colne. About You
Are you enthusiastic, compassionate, and committed to supporting people in a way that respects their dignity and individuality?
Do you have previous experience in care management and are looking for an opportunity where you can truly make an impact from day one?
To be successful in this role, you will have:
- Previous care management experience, ideally within a supported living environment.
- An NVQ Level 5 in Health & Social Care, be working towards it, or hold an equivalent qualification.
- Excellent interpersonal and communication skills, with the ability to create an inclusive and welcoming environment.
- The ability to provide inspiring, motivational, supportive, and effective leadership.
- A proactive and positive approach to managing people, quality, and service development.
- A full UK driving licence and access to a vehicle due to the needs of the service.

The Role
Working closely with the Registered Manager, the Scheme Manager will play a key role in the day-to-day running of our new supported living service, ensuring high-quality, person-centred support is delivered at all times. You will provide leadership and guidance to the team, promote independence for the people we support, and help maintain a safe, positive, and empowering environment.
Support is delivered 24 hours a day, 7 days a week.
Key responsibilities include:
• Supporting the Registered Manager with the day-to-day operation of the service, ensuring high standards of care and support are consistently achieved.
• Leading, supporting, and motivating staff teams to deliver person-centred support that empowers people to achieve their goals, aspirations, and greater independence.
• Ensuring support plans, risk assessments, and care records are accurate, up to date, and reflective of individual needs.
• Promoting the health, wellbeing, physical, emotional, cultural, and spiritual needs of the people we support, ensuring these are met in an inclusive and respectful way.
• Supporting with staff supervision, coaching, performance management, and the ongoing development of team members.
• Assisting with rota management, recruitment, staff inductions, and service compliance requirements.
• Building positive relationships with families, healthcare professionals, local authorities, and other external stakeholders.
• Supporting individuals to access social, educational, leisure, and community-based activities, helping them live fulfilling and meaningful lives.
• Monitoring quality and compliance, identifying areas for improvement, and working with the Registered Manager to implement positive changes.
• Ensuring all records, reports, and documentation are completed accurately and in line with company policies and regulatory requirements.
